About The Position

The CHW Team Lead supports analytical/clinical work within the ICM CHW program. This role serves as the subject matter expert for all CHW operations and collaborates with the ICM Manager to ensure the CHW program is run efficiently and effectively with clinical operations. The position works in collaboration with healthcare team members to support diverse populations by facilitating access to essential health services, equipment, and resources that promote health. Responsibilities include providing education on key health topics, supporting behavior change, screening for risk factors, and partnering with community organizations to deliver effective outreach and programming. The role also assists individuals in navigating public benefits, connecting with support services, and promoting positive health outcomes through coaching, education, and early intervention. This position works with a variety of grant-funded programs to bridge the gap between organizations, including health care delivery systems and community health services, to increase culturally relevant knowledge about health and wellbeing, improve underserved populations’ access to services, connect patients to community resources, and ultimately improve health outcomes.

Requirements

  • Bilingual in English and Spanish is preferred.
  • Experience working in a community-based setting preferred.
