About The Position

This is a full-time pharmacist position within our Anticoagulation and Pharmacotherapy Clinics, located across Snohomish County. The pharmacist will work directly with patients in the clinic to manage multiple conditions, including Anticoagulation, Dermatology, Diabetes, HCV, HIV/PrEP, GI, LAIAs, Lipids, and Osteoporosis. The successful candidate may be assigned to one or multiple of these services based on their background, experience, and clinic needs. The Senior Clinical Pharmacist will collaborate with physicians and staff to provide coordinated, comprehensive, high-quality care to patients. Requirements

  • Bachelor's Degree in PHARMACY from an accredited School of Pharmacy or PharmD in PHARMACY from an accredited School of Pharmacy.
  • Postgraduate Year One (PGY-1) Pharmacy Residency or equivalent education/experience.
  • Washington Pharmacist License upon hire.
  • Board of Pharmacy Preceptor License (Facility Managed) - when eligible and where applicable, upon hire.

Nice To Haves

  • Certification from Board of Pharmacy Specialties within 2 years of hire.

Responsibilities

  • Provide collaborative practice services in collaboration with the healthcare team.
  • Utilize registries for chronic diseases and reports/tools to identify target populations/patients and enhance therapeutic outcomes through disease/medication education and drug therapy optimization.
