About The Position

Join Community Health Network, an organization focused on providing exceptional care to its neighbors for over 60 years. As part of a progressive Ambulatory Care pharmacy team, you will participate in team-based care under approved protocols, making medication changes as appropriate. This role is an integral part of the interprofessional team, managing direct oral anticoagulants and warfarin through an innovative RN-PharmD hybrid anticoagulation service supporting Indianapolis and the central Indiana region. Care is delivered both in-person and virtually. Other responsibilities include providing staff education, guideline/policy development and implementation, and participation in performance improvement initiatives and practice advancement. You will have opportunities to mentor and teach pharmacy students and residents, and serve as a clinical resource for the healthcare team. Your expertise will advance patient care and achieve optimal outcomes. This position will provide patient care in the Anticoagulation Clinics and virtually. Expansion of cardiology pharmacist services to clinic sites in central Indiana and the Indianapolis region will provide an opportunity to advance services with the support, resources, and infrastructure of a well-established ambulatory cardiology pharmacy team.

Requirements

Strong knowledge of anticoagulation pharmacotherapy and management of hypercoagulable disease states, atrial fibrillation, venous thromboembolism, peripheral vascular disease, ischemic stroke.
Strong communication skills and a collaborative approach to patient care.
Practice at the top of your license utilizing established anticoagulation protocols.
Graduate of an accredited college of pharmacy, Bachelor or PharmD; Doctorate degree.
Indiana Pharmacist license.

Nice To Haves

Successful completion of a PGY1 Pharmacy Residency and PGY2 residency with Cardiology or Ambulatory Care focus is strongly encouraged.
A minimum of three (3) years working under a drug therapy agreement in a clinical setting.
Board certification as a Board Certified Pharmacotherapy Specialist, Board Certified Ambulatory Care Pharmacist, Board Certified Cardiology Pharmacist, or Certified Anticoagulation Care Provider is encouraged and required within 2 years of accepting the position.

Responsibilities

Manage direct oral anticoagulants and warfarin through an innovative RN-PharmD hybrid anticoagulation service.
Provide patient care in the Anticoagulation Clinics and virtually.
Provide staff education.
Develop and implement guidelines/policies.
Participate in performance improvement initiatives and practice advancement.
Mentor and teach pharmacy students and pharmacy residents.
Serve as a clinical resource for the healthcare team.
Advance the level of patient care to achieve the best possible outcomes.
Meet with patients individually (in-person and remotely) to provide extensive drug and disease state education, in particular peri-procedural management.
Work closely with physicians and health care teams, while providing direct patient care.
Work closely with patients to overcome medication adherence issues.
Assist patients in obtaining cost-effective anticoagulants with the assistance of medication assistance coordinators and Specialty Pharmacy colleagues.
Serve as a drug information resource for other providers and clinic staff, answering medication therapy questions.
Monitor for drug-therapy-related adverse events by performing/ordering the appropriate clinical or laboratory tests.
Engage in anticoagulation population health projects to impact the lives of patients served.
Provide timely consults for providers and RN collaborators.
Facilitate and support Anticoagulation Forum Center of Excellence standards.
