The College of Nursing is seeking a dedicated and experienced Senior Case Manager to join our Case Management Department supporting home and community-based waiver programs. This position plays a critical role in coordinating services for aging and disabled populations while helping individuals maintain independence and quality of life within their homes and communities. The Senior Case Manager will manage a full-time caseload and serve as an experienced resource within the department, providing guidance, support, and leadership to team members while ensuring high-quality case management services are delivered in accordance with organizational standards and program requirements. This position is ideal for a compassionate professional who is passionate about advocacy, care coordination, and improving access to healthcare and community resources. The successful candidate will work independently under limited supervision and will be responsible for all aspects of the case management process, including admissions, assessments and reassessments, service planning, monitoring, coordination of care, and discharge planning. The role requires strong critical thinking, communication, and organizational skills, as well as the ability to develop meaningful relationships with clients, caregivers, providers, and interdisciplinary teams. The Senior Case Manager will conduct comprehensive assessments by gathering in-depth information regarding a client's medical, social, environmental, and functional needs through interviews, documentation review, and collaboration with family members and service providers. Using this information, the position develops individualized and action-oriented case management plans designed to address identified healthcare and human service needs. Plans must include measurable goals, documented objectives, and coordinated interventions intended to support positive outcomes for clients receiving services through home and community-based programs. This role requires ongoing monitoring and evaluation of case management plans to determine effectiveness and progress toward established goals. The Senior Case Manager will organize, coordinate, and document the resources necessary to support clients, including healthcare services, community programs, caregiver support, and other interventions designed to improve quality of life and remove barriers to care. The position also evaluates outcomes related to clinical services, financial considerations, quality improvement measures, client satisfaction, and overall program effectiveness. Modifications to case management plans will be made as needed to ensure continued alignment with client needs and changing circumstances. In addition to direct case management responsibilities, the Senior Case Manager serves as a mentor and resource to other case managers and staff within the department. Responsibilities may include onboarding and training new or lower-level case managers, assisting with work assignments, monitoring staff performance, and participating in performance management activities. The position supports interdisciplinary collaboration and contributes to maintaining high standards of service delivery throughout the department. The Senior Case Manager also provides education and guidance to clients, caregivers, and families regarding healthcare conditions, available services, community resources, and strategies to support long-term wellbeing. This position may assist clients in navigating complex healthcare systems, accessing appropriate services, and addressing barriers that impact health outcomes and independence. Qualified candidates should possess professional experience in direct service provision, care planning, and working with aging and/or disabled populations. Certification in Case Management is preferred, along with experience in home-like or community-based settings.
