About The Position

Samaritan Daytop Village, a nationally recognized comprehensive Health and Human Services Agency with over 60 programs across New York City and the greater New York Area, is seeking a Senior Case Manager. This role is crucial for ensuring the smooth operation and coordination of all case management services, aligning with program goals, targets, and performance outcomes. The Senior Case Manager will provide essential case management and advocacy services to an assigned caseload and collaborate as a member of an interdisciplinary treatment team to support client recovery and rehabilitation.

Requirements

  • High School Diploma or Equivalent with Five (5) years of experience preferably supervising Case Managers within transitional/shelter programs or related setting OR Associates Degree in Human Services or related field with at least Two-Three (2-3) years of experience supervising Case Managers within transitional/shelter programs or related settings OR Bachelor’s Degree in Human Services with at least One-Two (1-2) years of experience supervising Case Managers within transitional/shelter programs or related settings.
  • Working knowledge of substance abuse treatment modalities and client self-help/support modalities.
  • Available to work a flexible schedule as needed in response to participant and staff needs.
  • Experience working directly with people from diverse racial, ethnic, and socioeconomic backgrounds.

Responsibilities

  • Coordinates, guides, and supervises activities of direct reporting staff.
  • Provides direct supervision of Case Managers.
  • Communicates all aspects of contract requirements to staff.
  • Leads and participates in administrative and staff meetings as required.
  • Troubleshoots problems and makes decisions in accordance with program policies, procedures, and protocols.
  • Monitors clients’ progress toward their goals and documents via progress notes.
  • Conducts periodic reviews of all case documentation to ensure accuracy, completeness, and clarity.
  • Conducts chart auditing of client files.
  • Directly manages a case portfolio of approximately 25 clients.
  • Participates in monitoring therapeutic environment to ensure that behavioral guidelines are followed, a therapeutic milieu is maintained and interpersonal relationships remain positive.
  • Provides individual counseling and facilitation of caseload group meetings and other assigned groups.
  • Provides psycho-education and counseling germane to chemical dependency and relapse prevention.
  • Administers appropriate behavioral interventions as needed.
  • Participates in assessment screenings to determine client needs/preferences; formulates and implements treatment, transition, and discharge/continuing care planning collaboratively with individual clients.
  • Attends case conferences and other meetings as scheduled.
