About The Position

Under the direction of the Contingency Management Program Manager, the Senior Case Manager will be responsible for leading the collaborative efforts of the Contingency Management Pilot mobile team in providing support and engaging participants enrolled in the Contingency Management Pilot. The pilot project supports participants to improve housing stability, health and other targeted behaviors through the use of case management services and incentives. The pilot is a collaborative effort of Department of Health Service’s Harm Reduction Division and Housing for Health, the Department of Public Health’s Substance Abuse Prevention and Control Harm Reduction team, and Brilliant Corners. In addition to the Senior Case Manager position, The Contingency Management mobile team consists of a Program Manager, a Peer Counselor, a Substance Use Counselor and a Nurse Practitioner. The team visits participants at their housing location, develops contingency management goals, assesses progress towards achieving those goals and issues incentives. The team will work closely with the participant’s Housing for Health Intensive Case Management Services Case Manager and property or facility management (as appropriate) to support the participant’s progress towards reducing problematic drug use and to retain housing for participants at-risk of losing their housing due to substance use related behaviors. Abstinence is not required to participate in the program.

Requirements

  • High School Diploma or GED or Equivalent experience serving target populations.
  • Extensive experience working with chronically homeless clients
  • Three (3) or more years of case management experience
  • Certification in substance abuse counseling. (CADC preferred).
  • Able to work in a high tolerance Harm Reduction model with high-acuity clients who have multiple barriers.
  • Detail-oriented with excellent time-management, organizational, written, verbal and computer skills
  • Self-starter who can lead a case management team in a face-paced work environment
  • Computer literacy, including knowledge of Microsoft Word, Outlook and Excel
  • Able to obtain and maintain CPR/First Aid certification

Nice To Haves

  • Bachelor’s degree in related field or equivalent experience.
  • Knowledge of the principles and practices of harm reduction, culture, terminology, and literature
  • Bilingual in English/Spanish a plus

Responsibilities

  • Support the Contingency Management Program Manager in providing orientation and training for new staff.
  • Conduct regularly scheduled quality assurance processes of documentation to ensure best practices.
  • Serve as a liaison between the team and external community partners.
  • Assist the Contingency Management Program Manager with trainings, program overviews and referral meetings, as needed.
  • Facilitate case conceptualization meetings and support team members with high acuity client cases.
  • Complete all necessary enrollment procedures and documentation for Contingency Management participants.
  • Provide orientation and guidance to Contingency Management participants and coordinate with staff to ensure collaboration on Contingency Management services.
  • Provide care by delivering and tracking all Contingency Management services, including accomplishing goals and documenting progress.
  • Provide field based safer drug use supplies, sexual health supplies, and harm reduction education to homeless individuals.
  • Act as an expert on the Harm Reduction Model by furthering the equity of our participants through harm reduction services by utilizing a diversity, equity, and inclusion lens.
  • Conduct a supportive services and resource needs assessment.
  • Attend regular agency and community meetings as it pertains to harm reduction service delivery.
  • Completes and enters all progress notes for service delivery, maintains compliance with due dates for documentation.
  • Effectively and professionally represent agency at community meetings.
  • Manage inventory of in-field incentives and distribution to participants.
  • Participate in case conferences with Housing ICMS providers.
  • Collaborate with each client to develop creative goal plans aimed at improving overall wellbeing and housing stability.
  • Develop and maintain a complete, accurate, and current client file with all required documents and data in agency records and electronic databases.
  • Document all client activity in accordance with funder expectations.
  • Coordinate with DHS and community partners to ensure clients are connected to primary health care and insurance to reduce need for emergency health care services.
  • Maintain confidentiality of client files per HIPAA and all applicable guidelines.
  • May be required to drive program vehicle (Sprinter van).
