Requirements

  • Knowledge of and belief in “Housing First” philosophy and strategies.
  • Passion for and demonstrated experience successfully supporting vulnerable populations, especially homeless clients with mental disabilities.
  • BA degree in social work or related field; equivalent experience will be considered.
  • Minimum of two years’ experience in the human service field, preferably with housing retention barriers such as substance abuse, mental health, and/or income challenges.
  • Outstanding written and verbal communication skills.
  • Computer proficiency in the use of Microsoft and database applications.
  • Must have an operational vehicle, auto insurance, valid California driver’s license, and be willing to drive for work.
  • Must pass LiveScan screening and TB test.
  • Ability to drive an automobile and transport oneself and consumers to appointments or meet at consumer homes, including driving at night.
  • Must be able to work in a desk setting for up to 3 hours at a time to make calls to consumers, research resources on the internet, and collaborate with community providers with calls/emails.
  • Office setting activities are required, such as typing on a laptop/computer to respond to emails, entering case notes into a database, and copying/faxing forms and documents on a copy machine related to maintaining or obtaining housing.
  • Must be able to climb stairs for home visits to consumers in second or third-floor apartments.
  • Ability to accompany consumers on walks to local resources up to ½ mile in distance.
  • Candidates must be authorized to work in the USA without the need for sponsorship.

  • Bi-lingual in English/Spanish preferred.
  • Occasionally must be able to lift and carry objects weighing up to 25 pounds, including furnishings for new residents, files, holiday gifts, and food donations.

Responsibilities

  • Provide home-based case management and supportive mental health services for formerly chronically homeless families with special needs.
  • Conduct psychosocial, life skills, and financial assessments.
  • Develop and support the implementation of family service plans.
  • Facilitate linkage to community and mainstream resources for stable housing and self-sufficiency.
  • Assist the Program Manager with onboarding new staff, including training on Standards of Practice, documentation, and HMIS/Apricot usage.
  • Provide shadowing opportunities for new staff in fieldwork, assessments, intakes, and client interactions.
  • Train and assist staff with program-specific paperwork, contract liaison forms, and filing processes.
  • Maintain relationships with contract liaisons and referral sources, acting as a point of contact in the manager's absence.
  • Review, generate, and customize reports in HMIS/Apricot to support team work and monitor program outcomes.
  • Manage and track subsidy requests and collaborate on data reviews.
  • Participate in quarterly data and finance meetings.
  • Provide ongoing support, guidance, and feedback to staff.
  • Serve as the point of contact when the manager is unavailable.
  • Attend community and external meetings on behalf of Hope Solutions, creating agendas, hosting discussions, and taking notes.
  • Maintain awareness of culturally diverse consumer populations and adapt services accordingly.
  • Maintain contact with consumers and provide home visits based on identified needs.
  • Work collaboratively with consumers to develop and monitor progress on service plans.
  • Provide Case Management support, including budgeting, accessing benefits and services, and accompanying consumers to appointments.
  • Assure the provision of supportive mental health interventions and crisis management.
  • Provide linkage to medical, mental health, addiction/recovery, financial, legal, and other community services.
  • Assist consumers in locating housing units and negotiating issues with landlords/property managers.
  • Collaborate with providers to overcome issues that may place consumers at risk for eviction.
  • Assist with consumer certification/verification paperwork.
  • Represent Hope Solutions professionally in all circumstances.
  • Comply with legal/ethical professional guidelines for consumer confidentiality, HIPAA, consumer rights, and safety.
  • Comply with all local, state, and federal regulations and agency policies.
  • Complete required administrative paperwork and reports accurately and in a timely manner.
  • Attend all required meetings.
  • Complete personnel-related paperwork and expenditure reports.
  • Document all services provided in a timely manner.
  • Assist with data generation for reports and funding applications.
  • Perform other duties and responsibilities as assigned.
  • Be available for on-call, after-hours, and backup work as required.
