About The Position

In accordance with Division of Mental Health and Addictions Services (DMHAS), Commission on Accreditation of Rehabilitation Facilities (CARF), Medicaid, and Volunteers of America policies and standards, the Senior Case Manager is responsible to deliver wellness and recovery-oriented case management and support services to service recipients. The Senior Case Manager provides support directly to the residents, including but not limited to: skill development, supportive counseling, transportation, crisis intervention, medication monitoring, assistance with task completion, vocational and social support. The Senior Case Manager actively supports the Residence Manager/Director in the management of the program’s staff and resident treatment. The Senior Case Manager is available to cover open shifts when necessary to ensure adequate coverage at the facility at all times. Our program serves consumers with psychiatric disabilities living in their own apartments in a senior housing complex.

Requirements

  • Bachelor’s degree from an accredited college or university in a mental health, health or other appropriate human services related discipline and one-year of experience; or satisfactory combination of education and experience.
  • Valid state driver’s license with a safe driving record that meets agency procedures.

Responsibilities

  • Provide on-site services and support to service recipients.
  • Communicate observations and information about the service recipient to the residence manager, RN and treatment team.
  • Supervise, organize and support recreational and/or socialization activities.
  • Transport and/or assist service recipients with accessing transportation to appointments.
  • Provide residential counseling and crisis intervention (but not including crisis intervention counseling).
  • Participate in the development of the individual service plan.
  • Participate in individual service coordination activities, including vocational and educational services.
  • Document observations, information and services provided to the service recipient in progress notes.
  • Maintain a clean, safe environment in the facility, including assisting with meal preparation as needed and working with counselors assisting them with maintaining the safety and cleanliness of the facility per licensing and VOA standards.
  • Take the lead in onboarding, training, orientation of new staff to the program.
  • Responsible for maintaining the scheduling of staff on a weekly basis to ensure all shifts are covered and adequate coverage is provided.
  • Responsible for documenting in the EHR, the Weekly Attendance Verification Note (WAVN) after confirming that the nightly absence calendar is completed for the week to ensure that the attendance of all service recipients is accurate and up to date.
