About The Position
The Senior Case Manager engages directly with guests, assesses needs, makes appropriate referrals or provides supportive services to stabilize medical and mental health conditions in order to prepare residents for more independent living.
Requirements
- Bachelor’s Degree in Social Work or a related field required.
- Experience in case management, strong understanding of homelessness, substance abuse and mental health issues.
- Knowledge of the recovery process and drug and alcohol inpatient and outpatient programs.
Nice To Haves
- Master’s Degree preferred.
Responsibilities
- Provide an assessment of each guest’s needs and oversee the creation of an individualized Housing Plan.
- Demonstrate a thorough understanding of the Safe Haven Model and the application of Harm Reduction in all guest interactions and Housing Plan interventions.
- Conduct Shelter Intake Interviews with potential candidates.
- Meet with the CSP Program Coordinator and shelter staff (formally and informally) to identify guests in need of case management services.
- Assist guests in maintaining important relationships and serve as an advocate with service providers.
