About The Position

Under direct supervision, serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community. The Senior Case Manager has a proven track record as a subject matter expert in case management. This position applies their expertise as a mentor in the program and supports the function by contributing their expertise in facilitation of morning meetings, helping other Case Managers organize and implement member appointments, conduct training sessions related to case management, specifically addressing more complex situations, and assisting in daily touch points to ensure staff are meeting service billing expectations.

Requirements

  • A Bachelor’s Degree (in social services or human services) and two years of direct care service in Mental Health or community services or a High School Graduation or G.E.D. equivalent and five (5) years of direct services in Mental Health or community services; an Associates degree may be substituted for two (2) years of experience.
  • Proficiency in Case Management: Demonstrated excellence in case management tasks, including meeting or surpassing direct care service billing expectations. Proven ability to navigate and optimize direct care service billing processes.
  • Case Management Subject Matter Expert: Proven track record and demonstrated excellence as a subject matter expert in case management, documentation, member engagement, outreach, and de-escalation. In-depth knowledge and application of best practices in the aforementioned areas.
  • Exemplifies support of positive work culture.
  • Proven ability to build effective relationships with team members, stakeholders, and clients.
  • Demonstrated involvement in training initiatives, with a track record of successfully training others. Proactive assistance with onboarding and orientation of new hires.
  • Sensitivity to multi-cultural populations and issues is required.
  • Must be at least 18 years of age.
  • Must be CPR, First Aid, and CPI certified (hands-on course) on date of employment or prior to providing direct client care and maintain current certification throughout employment.
  • All opportunities at Telecare are contingent upon successful completion and receipt of acceptable results of the applicable post-offer physical examination, 2-step PPD test for tuberculosis, acceptable criminal background clearances, excluded party sanctions, and degree or license verification.
  • If the position requires driving, valid driver license, a motor vehicle clearance and proof of auto insurance is required at time of employment and must be maintained throughout employment.
  • Valid and current driver’s license, and personal vehicle insurance with your name listed as a driver.
  • Willingness to use your personal vehicle to drive clients to appointments and groups, etc. (weekly mileage reimbursement at the IRS rate)
  • Willingness to use the company vehicle to drive clients to appointments and groups, etc.

Responsibilities

  • Facilitate morning meetings.
  • Help other Case Managers organize and implement member appointments.
  • Conduct training sessions related to case management, specifically addressing more complex situations.
  • Assist in daily touch points to ensure staff are meeting service billing expectations.
  • Provide case management services to adults, aged 18 and older, who utilize emergency rooms, inpatient settings, and crisis services to regularly manage their mental health and substance use needs.
  • Utilize a 'whatever it takes' outreach and engagement approach to help reduce the need for inpatient and emergency room care while improving mental health outcomes for members served by connecting members to supports and resources available within their community.
  • Provide linkages, supports, and resources to help community members remain in the least-restrictive, most self-sufficient, and recovery-oriented settings possible.
