About The Position

Waukesha County’s Department of Health and Human Services is seeking a Service Coordinator to fill a position serving children who are enrolled in the Children’s Long-Term Support (CLTS) Waiver Program. This position provides comprehensive case management and service coordination to children diagnosed with a wide range of physical, developmental, and mental health disabilities, as well as support to their families. The Service Coordinator works collaboratively with children, caregivers, providers, schools, medical professionals, and community partners to ensure coordinated, high-quality services that enhance each child’s quality of life and support children remaining safely in their homes and communities. This role also includes providing advocacy, guidance, and information regarding available programs, county and community resources, and Children’s Long-Term Support (CLTS) Waiver services. Successful candidates will have working knowledge of CLTS, Medicaid, county and community resources, and best practices in service coordination. Strong case management skills—including the ability to multitask, prioritize competing needs, maintain organization, work independently, and thrive in a collaborative, fast-paced environment—are essential. Candidates must demonstrate strong relationship-building skills and the ability to effectively collaborate with families, caregivers, service providers, schools, and community agencies. Additional responsibilities include participation in ongoing professional development activities such as trainings, staff meetings, and seminars; adherence to Department of Health Services (DHS) guidelines and state requirements; fulfillment of emergency government responsibilities as assigned during a Waukesha County Emergency Government declaration; and completion of other duties that support the department’s mission and the well-being of children and families. This is a 40-hour per week position, Monday through Friday, 8:00 a.m. to 4:30 p.m., with the flexibility to adjust your schedule as needed to meet the needs of families, including occasional evening visits. Remote work is available for up to 50 percent of the pay period after required training and performance expectations are met. Position will be posted with a two-week deadline.

Requirements

  • A bachelor’s degree from a recognized college or university in sociology, social work, psychology, nursing, criminal justice, or a closely related field.
  • Two years of responsible work experience in social work, counseling, nursing, criminal justice, or a closely related field (for Senior Care Manager).
  • One year of related work experience in social work, counseling, nursing, criminal justice, or a closely related field (for Care Manager).
  • Valid driver’s license.

Nice To Haves

  • A master’s degree in social work, nursing, psychology, criminal justice, or a closely related field may substitute for one year of the work experience.
  • Social work certification may substitute for one year of work experience.

Responsibilities

  • Completing functional assessments
  • Developing, implementing, and monitoring individualized service plans and outcomes
  • Coordinating services across various ages and disability types
  • Conducting regular home visits and monthly family contacts
  • Maintaining communication with families, providers, and community partners to monitor progress and ensure timely access to needed resources and supports
  • Providing advocacy, guidance, and information regarding available programs, county and community resources, and Children’s Long-Term Support (CLTS) Waiver services
  • Participation in ongoing professional development activities such as trainings, staff meetings, and seminars
  • Adherence to Department of Health Services (DHS) guidelines and state requirements
  • Fulfillment of emergency government responsibilities as assigned during a Waukesha County Emergency Government declaration
  • Completion of other duties that support the department’s mission and the well-being of children and families
