About The Position

Deliver care management services administratively and clinically to an assigned caseload of clients with mental illness, substance use disorders and/or co-occurring disorders. Given your advanced knowledge, you will provide mentorship and train new staff and work with Leadership on special projects.

Requirements

Three years of experience working in Human Services, particularly in mental health, substance use, or developmental disability field
Two years of experience as a Care Manager or comparable role
Valid driver's license and use of a personal vehicle

Responsibilities

Provides direct services and support to clients
Care coordination
Provide mentorship and training to Care Managers
Assists with on-boarding requirements
Complete projects to support program development, data collection and analysis
