About The Position

Metrolinx is connecting communities across the Greater Golden Horseshoe, operating GO Transit and UP Express, and the PRESTO fare payment system. They are also involved in building new rapid transit, including GO Expansion, Light Rail Transit routes, and major expansions to Toronto’s subway system. Metrolinx is an agency of the Government of Ontario, committed to equity, diversity, and inclusion. The Safety Standards & Practices Team within the Engineering and Safety Division is seeking a Senior Accident Investigator to manage and lead investigations into safety-related accidents and incidents. This includes deploying to sites, gathering evidence, conducting tests, performing detailed analysis, preparing safety investigation reports, and recommending corrective actions to improve safety.

Requirements

  • Completion of a degree in Safety, Engineering, Human Factor or a related discipline - or a combination of education, training, and experience deemed equivalent.
  • Demonstrated training/experience in safety planning, operational risk assessment, human factors, regulatory compliance and/or accident investigation within the construction/transportation field.
  • Strong analytical and critical‑thinking skills, with the ability to synthesize complex technical and operational information into clear findings and actionable recommendations.
  • Demonstrated ability to navigate complex organizational environments and manage diverse stakeholder interests.
  • Proven judgment and decision‑making capability in high‑risk or sensitive situations.
  • Strong interpersonal and communication skills, with the ability to influence, build trust, and engage effectively with senior leadership and cross‑functional teams.
  • Ability to handle confidential and high‑profile investigations with professionalism, discretion, and diplomacy.
  • Must be able to obtain qualifications in Canadian Rail Operating Rules (CROR), offered through Metrolinx upon the start of employment.
  • A valid Ontario Class G driver’s license, to be maintained in good standing, is required.

Nice To Haves

  • Experience in construction incident investigations is an asset.
  • Canadian Registered Safety Professional (CRSP) or Certified Safety Professional (CSP) designation is an asset.

Responsibilities

  • Lead and oversee the end‑to‑end incident investigation process, coordinating internal teams, technical experts, and external stakeholders to ensure timely and defensible outcomes
  • Develop and manage investigation work plans using the Metrolinx investigation methodology, including defining scope, milestones, roles, and deliverables.
  • Provide technical leadership in the development of investigation findings, conclusions, and safety recommendations, ensuring clarity, accuracy, and consistency.
  • Conduct root cause analysis and safety trend analysis to identify systemic, organizational, technical, and human factors contributing to incidents.
  • Prepare and deliver investigation briefings and status updates to senior leadership.
  • Present investigation findings, lessons learned, and corrective action recommendations to senior leaderships and review committees to support decision‑making and policy or program improvements.
  • Ensure compliance with Transportation Safety Board of Canada (TSB) reporting requirements and act as the organizational lead for coordination during TSB‑related investigations.
  • Support the development, maintenance, and continuous improvement of investigation methodologies, tools, and training.
  • Provide subject‑matter expertise and support on safety investigations and related initiatives as assigned.
