Requirements

• Strong communicator, both written and verbal, who easily connects with people.
• Ability to assess needs and create action plans that drive real results.
• Experience working with multiple population types and advocating for individuals.
• Problem-solving and conflict resolution skills.
• Ability to work as a team player and independently.
• Sensitivity and understanding towards cultural and economic differences.
• Minimum of two (2) - four (4) years of education and/or experience in a related field required.

Nice To Haves

• A four-year post-secondary degree in Human Services is preferred.

Responsibilities

• Provide case management services that help individuals and families set and achieve personal goals.
• Conduct assessments, develop individualized plans, and assist in stabilizing housing situations.
• Keep detailed records, document all contacts, and maintain participant confidentiality.
• Support participants in becoming independent from public assistance.
• Build relationships with service providers, attend networking meetings, and advocate for participants.
• Ensure accurate documentation, track referrals, and complete necessary reports on time.
