About The Position
We are looking for a proactive employee to provide person-centered services to support and assist individuals in Self-Direction services to achieve independence and maintain their health and safety.
Requirements
- Basic word processing and e-mail skills
- Must possess a valid driver’s license and transportation
- Must be able to demonstrate direct care duties including personal assistance
Nice To Haves
- Experience working with people with disabilities
Responsibilities
- Assistance with personal needs and activities of daily living (ADL)
- Monitoring health and safety, building advocacy, decision making, and community connections
- Administer medication and treatment plans
