The purpose of this position is to manage the Special Diabetes Program for Indians (SDPI) Grant and coordinate program activities. This also includes providing patient care specifically to the Sage Memorial Hospital diabetic population and promoting comprehensive diabetes care for patients. This position is responsible for promoting SDPI Best Practices and a systematic approach to diabetes care. Under general supervision of the Chief Community Health Officer, Community Health Clinical Leadership and in collaboration with the OPD Medical Director, accomplishes the Special Diabetes Program for Indians (SDPI) program’s strategic objectives by planning, organizing, and implementing the assigned functions required to operate and maintain departmental activities and services. Ensures quality, patient care delivery and maintains confidentiality of all privileged information. This position is designated as a working supervisor position, such that the supervisor is directly involved with provision of quality patient care to assist with obtaining direct insight in quality improvement opportunities and implementation of quality initiatives within the department being supervised.
