About The Position

The purpose of this position is to manage the Special Diabetes Program for Indians (SDPI) Grant and coordinate program activities. This also includes providing patient care specifically to the Sage Memorial Hospital diabetic population and promoting comprehensive diabetes care for patients. This position is responsible for promoting SDPI Best Practices and a systematic approach to diabetes care. Under general supervision of the Chief Community Health Officer, Community Health Clinical Leadership and in collaboration with the OPD Medical Director, accomplishes the Special Diabetes Program for Indians (SDPI) program’s strategic objectives by planning, organizing, and implementing the assigned functions required to operate and maintain departmental activities and services. Ensures quality, patient care delivery and maintains confidentiality of all privileged information. This position is designated as a working supervisor position, such that the supervisor is directly involved with provision of quality patient care to assist with obtaining direct insight in quality improvement opportunities and implementation of quality initiatives within the department being supervised. Requirements

Bachelor’s Degree in Nursing from an accredited school of nursing, with 2 years of experience in Diabetes care.
Must possess a current, unrestricted RN license from Arizona or other compact state OR Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN).
Minimum 2 years-experience providing direct care to patients required.
Must possess valid BLS certification.
Must be able to successfully pass a pre-employment drug/alcohol screen, background investigation and meet all employee health requirements.
Professional knowledge and skill with diabetes education and role as a health educator.
Ability to investigate and analyze information and to draw conclusions.
In-depth knowledge and understanding of the nature of diabetes, related issues and patient care concerns.
Ability to design, develop, deliver, and evaluate educational programs and materials.
Ability to provide technical advice, guidance, and support to professional staff in area of specialty
Knowledge of effective principles and practices of patient education and development
Knowledge of clinical practice and patient care
Knowledge of patient evaluation and triage procedures
Knowledge of Diabetes Program grant requirements & deadlines
Knowledge of clinical instruments and equipment
Knowledge of appropriate procedures and standards for administering of medications and patient care aids

Nice To Haves

Two (2) years of experience in case management / care coordination preferred.
Must have Certified Diabetes Care and Education Specialist (CDCES) credential or obtain within two years of employment.
Ability to speak the Navajo language and/or familiarity with the Navajo Way, preferred.

Responsibilities

Supervision of SDPI staff
Reporting for the SDPI grant
Oversight and management of goals, objectives and outcomes of SDPI Best Practices and Required Key Measures (RKM)
Conduct assessments & evaluations of diabetic patients
Identify high risk diabetic patients and assists in developing clinical care plans in collaboration with and approval of PCMH PCP and RN Coordinator
Educates patients on the necessary lifestyle changes involved in a diabetes diagnosis, including nutrition, medication, physical activity and glucose testing.
Provide standardized, appropriate educational material and learning opportunities to the patient, family and/or significant other concerning diagnosis and recommended treatment, including, but not limited to self-injection training, home glucometer use, basic nutrition, medication administration, home medication, patient involved care planning, and one-to-one education.
Practice infection control and patient safety procedures.
Conduct Medical Nutrition Therapy (if licensed)
Administer medications/medical treatment, monitors patient condition, and response to therapy in accordance with current standard of care.
Collects patient data to monitor health status and proactively manage the patient's condition.
Ensures data is gathered and accurately documented demonstrating the utilization of the appropriate process to promote patient outcomes.
Work closely with professional providers and other clinic staff in planning and evaluating patient needs and care.
Conducts follow-up with patients.
Assists patients in developing and meeting goals and areas of need and assist in developing treatment plans and assesses them regularly.
Assess patients’ formal and informal support systems; evaluate effectiveness of services provided, treatment and medication.
Assist with development and coordination of patient care programs, including, but not limited to, support groups, outreach events, patient group education opportunities, etc.
Independently manages activities of caseload
Ensure proper provision of care to provide for physiological, psychosocial environmental, educational, and discharge planning needs of patient.
Participate in community education events at the chapter level and at the hospital.
Perform health screenings and assist in updating the diabetes registry of all diabetic patients who receive care at SMH.
Review, revise, and create department policies and procedures for the Diabetes Program
Complete employee evaluations and competency forms accordingly and within the required time frame.
Maintain compliance with applicable laws, regulations, and ordinances including Joint Commission Accreditation.
Initiate and maintain professional relationships with external resources and facilities to establish efficient patient care delivery.
