The Program Coordinator has the primary responsibility for the development/expansion of School Health Centers (SHCs) within the Rural Health Transformation Program (RHTP). This includes but is not limited to implementation, execution, monitoring and evaluation of grant deliverables for School Health Centers within the RHTP. School Health Centers increase access to primary and preventive health care that improves the health status and academic performance of older children and adolescents, ages 10 to 19 years. The RHTP will expand access to these services in identified rural communities within North Carolina. THIS POSITION IS FOR A 5-YEAR RURAL HEALTH TRANSFORMATION GRANT. NC DHHS IS REQUIRED TO REAPPLY EACH YEAR FOR CONTINUED FUNDING DURING THE 5-YEAR GRANT PERIOD.
