The Licensed Practical Nurse (LPN) is responsible to the Director of Nursing for rendering nursing care in assigned areas. This role involves utilizing nursing techniques, procedures, and established standards based on the scope of practical nursing. The LPN will perform routine administrative and clinical tasks, provide quality care to patients by supporting CCWV quality initiatives, and contribute to developing patient/caregiver plans of care. The position requires carrying out physician/mid-level provider orders accurately, documenting pertinent observations and interventions, and performing selected tasks related to patient care and the patient's environment safely. This role also involves maintaining and coordinating communication among patient/caregiver, staff, and provider, assisting in effective communication with patients/caregivers, and reporting patient needs/problems and observations to the provider. Collaboration with other healthcare team members is essential. The LPN will provide patient/caregiver health teaching as delegated, use appropriate teaching methods, and document health teaching and patient/caregiver response. Attendance and participation in staff development programs and meetings are required. The LPN will place patients in examining rooms, prepare them for examination, and may assist providers. Obtaining patient medical history, recording vital signs, and asking/documenting responses to health and social history questions are key duties. Evaluating Social Determinants of Health responses, advising the provider of positives, and connecting patients to resources are also part of the role. The LPN will annotate patient charts, contact patients with findings, and perform accurate documentation. Daily equipment checks, cleaning and sterilizing instruments, maintaining the stock room, and monitoring medication/vaccination inventory are also responsibilities. Counseling patients on medication refills and answering clinical calls are expected. The LPN will also assist in the orientation/training of new team members, work cooperatively with fellow team members, and assist others. Problem-solving involves demonstrating sound judgment, investigating unusual orders, following proper reporting procedures, and recording/reporting the need for service maintenance or repair of equipment. Evaluating work, taking initiative to redo inadequate work, and ensuring compliance with regulatory standards are also important. Productivity and efficiency are demonstrated by responding to changing situations, minimizing non-productive time, organizing job functions, and taking initiative to make decisions without direct supervision. Adherence to departmental policies, including infection control and Body Substance Isolation (BSI), is mandatory.
