The Licensed Practical Nurse (LPN) is responsible to the Director of Nursing for rendering nursing care in assigned areas. This role involves utilizing nursing techniques, procedures, and established standards based on the scope of practical nursing. The LPN will perform routine administrative and clinical tasks, provide quality care to patients by supporting CCWV quality initiatives, and contribute to developing the patient/caregiver plan of care. This position requires performing delegated independent nursing functions, carrying out provider orders accurately, and documenting pertinent observations and interventions. The LPN will also be responsible for administering medications, performing venous blood draws and IV insertion (after training), and maintaining effective communication among the patient/caregiver, staff, and provider. Health teaching will be provided as delegated, and the nurse will attend staff development programs and meetings. Additionally, the LPN will prepare patients for examinations, obtain medical history and vital signs, evaluate Social Determinants of Health responses, and connect patients to resources. The role involves annotating patient charts, performing daily equipment checks, cleaning and sterilizing instruments, maintaining the stock room, and monitoring medication/vaccination inventory. The LPN will counsel patients on medication refills and questions, answer clinical calls, and perform other duties as assigned. Patient satisfaction, teamwork, problem-solving, efficiency, and adherence to departmental policies are key aspects of this role.
