About The Position

The School-Based Health Registered Nurse (RN) serves as a key member of CHI's School-Based Health Program and is responsible for delivering, coordinating, and promoting comprehensive health services for students within assigned schools. The RN works collaboratively with students, families, school personnel, healthcare providers, behavioral health professionals, and community partners to improve student health outcomes and support academic success. The RN provides direct nursing care, health assessments, care coordination, chronic disease management, preventive health services, telehealth support, health education, and case management. The RN serves as a clinical resource within the school environment and helps ensure students receive timely, high-quality, patient-centered care.

Requirements

  • Graduate of an accredited School of Nursing.
  • Associate Degree in Nursing required.
  • Current Florida Registered Nurse license in good standing.
  • Current Basic Life Support (BLS) certification through the American Heart Association.
  • CPR certification required.
  • Minimum of one (1) year of nursing experience required.
  • Strong clinical assessment and triage skills.
  • Knowledge of pediatric and adolescent health issues.
  • Ability to work independently and exercise sound clinical judgment.
  • Strong communication, interpersonal, and organizational skills.
  • Ability to collaborate effectively with students, families, school personnel, and healthcare providers.
  • Proficiency in electronic health record documentation.

Nice To Haves

  • Bachelor of Science in Nursing (BSN) preferred.
  • Experience in pediatrics, family medicine, community health, public health, school health, emergency care, or ambulatory care preferred.
  • Experience working with pediatric and adolescent populations preferred.
  • Experience in a Federally Qualified Health Center (FQHC), school-based health program, public health department, or community health setting preferred.
  • Bilingual English/Spanish and/or English/Haitian Creole preferred.

Responsibilities

  • Provide nursing assessments, triage, and direct care services for students presenting with illness, injury, or other health concerns.
  • Assess student health status and determine appropriate interventions, referrals, or escalation of care.
  • Monitor students with acute illnesses and injuries and provide follow-up as appropriate.
  • Administer medications and treatments in accordance with physician orders, standing orders, CHI policies, and applicable regulations.
  • Respond to urgent and emergency situations and initiate appropriate interventions.
  • Develop, implement, and monitor individualized care plans for students with chronic health conditions.
  • Coordinate care for students with asthma, diabetes, seizure disorders, sickle cell disease, behavioral health conditions, and other chronic illnesses.
  • Monitor compliance with treatment plans and assist students and families in overcoming barriers to care.
  • Collaborate with providers, families, and school personnel to support student health and safety.
  • Conduct health screenings including blood pressure, height, weight, BMI, vision, and other approved screenings.
  • Identify students requiring additional evaluation, follow-up, or referral services.
  • Promote preventive health practices and healthy lifestyle behaviors.
  • Support oral health initiatives, fluoride varnish programs, immunization compliance efforts, and wellness activities.
  • Coordinate referrals to medical, dental, behavioral health, vision, specialty care, and community-based services.
  • Assist students and families in navigating healthcare systems and accessing needed services.
  • Monitor referral completion and support closed-loop referral processes.
  • Collaborate with care coordinators, community health workers, and healthcare providers to ensure continuity of care.
  • Facilitate telehealth visits between students and CHI medical and behavioral health providers.
  • Prepare students for telehealth encounters and provide clinical support during virtual visits.
  • Assist with implementation of RN telehealth services when onsite clinical staff are not available.
  • Ensure telehealth activities comply with privacy, confidentiality, and regulatory requirements.
  • Collaborate closely with behavioral health professionals to identify students requiring behavioral health services.
  • Participate in multidisciplinary care planning and case discussions.
  • Support behavioral health screening, referral, crisis intervention, and follow-up activities.
  • Assist in identifying students with social, emotional, or behavioral health concerns and facilitate appropriate interventions.
  • Serve as a liaison between CHI, students, families, and school personnel.
  • Develop collaborative relationships with school administrators, counselors, teachers, social workers, and school nurses.
  • Participate in school meetings, multidisciplinary team meetings, and student support activities as assigned.
  • Promote awareness and utilization of school-based health services among students and families.
  • Support student advocacy efforts that improve access to healthcare and educational success.
  • Maintain accurate, timely, and complete documentation in the electronic health record (EPIC).
  • Ensure compliance with HIPAA, FERPA, CHI policies, grant requirements, and applicable federal and state regulations.
  • Participate in quality improvement initiatives, documentation audits, chart reviews, and performance improvement activities.
  • Monitor clinical outcomes, utilization data, and performance metrics as assigned.
  • Assist with student registration and consent processes as needed.
  • Maintain clinical supplies, equipment, and inventory at assigned school sites.
  • Ensure compliance with infection prevention and control standards.
  • Participate in staff meetings, training programs, competency assessments, and professional development activities.
  • Support implementation of new initiatives, workflows, and operational improvements within the School-Based Health Program.
  • Demonstrate professionalism, accountability, cultural sensitivity, and commitment to student-centered care.
  • Maintain confidentiality and protect student and family privacy at all times.
  • Perform other duties as assigned.
