About The Position

The HealthCare Connection (THCC) is seeking a School-Based Behavioral Health Provider for a unique opportunity to serve children, adolescents, and families within our School-Based Health Centers. This is a rewarding role ideal for a licensed clinician eager to make a lasting impact on the underserved communities through integrated care. As a School-Based Behavioral Health Provider, you will deliver brief, solution-focused assessments and interventions aimed at functional restoration. You will collaborate closely with primary care providers to develop and implement clear treatment plans, provide behavioral health services including crisis intervention and substance abuse screenings, and educate and consult with the primary care team on mental health, chemical dependency, and behavioral medicine. You will also support patients and families with self-management, behavior change, and harm reduction strategies, identify referral resources and coordinate care for patients requiring specialized services, and maintain thorough documentation in Electronic Medical Records (EMR).

Requirements

  • Master’s degree in Clinical Social Work, Counseling, or Psychology.
  • Licensed in the State of Ohio (LISW, LPC, LPCC, LMFT).
  • Three (3) or more years of experience in behavioral health, preferably in an integrated primary care setting.
  • Strong knowledge of population-based healthcare, behavioral medicine, and evidence-based treatment models.
  • Experience working with diverse, multicultural populations.

Nice To Haves

  • Bilingual (Spanish/English) strongly preferred.

Responsibilities

  • Conduct comprehensive bio-psychosocial assessments addressing mental health, substance use, co-occurring disorders, and medical needs.
  • Manage psychosocial aspects of chronic and acute diseases in collaboration with the primary care team.
  • Be immediately available during working hours to assist with crisis interventions and psychiatric emergencies.
  • Provide individual, group, and family interventions as needed within the school setting.
  • Use electronic tracking tools to monitor treatment outcomes and ensure effective care coordination.
  • Educate staff, patients, and families on mental health best practices.
