About The Position

This role involves coordinating and scheduling hospice aide visits to ensure patient care plans are met and staff availability is managed effectively. The coordinator will utilize the EMR system, communicate with the Patient Care Manager, and maintain accurate scheduling records. Responsibilities also include managing staff schedules for various leave types, tracking staff punctuality and reliability, and ensuring compliance with all relevant regulations and policies.

Requirements

  • High School Diploma Preferred
  • At least one year experience, preferred, as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice, or long term care facility or completion of Certified Nursing Assistant or Certified Home Health Aide Program.
  • One year experience as an aide scheduler preferred.
  • Demonstrates good communication, interpersonal, negotiation and public relations skills
  • Demonstrates organization, assertiveness, flexibility, cooperation in performing job responsibilities
  • Must have current driver's license
  • Must have reliable transportation and provide ongoing valid and current auto liability insurance

Nice To Haves

  • If an aide, has successfully completed one of the following: Training program/competency eval as specified in §418.76(b)(c)of the Medicare regulations, Competency eval program that meets §418.76(c), Nurse aide training/competency eval program approved by the state and listed in good standing on the state nurse aide registry, State Licensure program that meets requirements of §418.76(b)(c)
  • CHPNA certification a plus
  • Experience in EMR a plus
  • CNA preferred.

Responsibilities

  • Utilize the EMR software system to schedule clinical staff patient visits
  • Schedule patient visits according to care plans and staff availability
  • Confer with Patient Care Manager and/or Clinical Supervisor on problems and complaints timely
  • Update and maintain the schedules on a regular basis throughout the day, enter scheduling data, create schedules
  • Provide staffing scheduling for sick leave, vacation, long term leave
  • Verify documentation of information such as completion of visits, frequency & duration of visits
  • Schedule new patients for identified services and maintains a close connection with Admissions/Intake for communication of admission needs and staff availability
  • Ensure scheduled visits are entered timely
  • Order and maintain patient supply inventory; possibly help aides load in their cars
  • When frequency is incorrect or missed, notify PCM
  • Oversight and tracking of staff in regard to punctuality, assignments and reliability
  • Communicate and report to Patient Care Manager regarding tardiness, unplanned, unscheduled Aide Call Ins and other confidential findings
  • Timely submission of all documentation
  • Conduct all business activities in a professional and ethical manner
  • Performance of other duties as required
  • Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
