The Scheduler/Intake Coordinator coordinates client’s transition from an institutional setting to home health care services, processes client referrals and initiates services. This role operates under the general supervision of the Director of Clinical Services and adheres to organizational policies, federal, and state regulations. Essential functions include collecting database information during intake, assisting clients and referral sources in establishing home care/hospice plans, communicating and educating referral sources, scheduling staff for various programs (Medicare, Private Duty, Passport), serving as a liaison between referral sources and interdisciplinary teams, investigating and documenting payor coverage, advocating for client benefits, tracking referrals, updating referral sources on client status, and communicating necessary information to team members regarding new admissions and referrals. The role also requires providing appropriate documentation of referrals per agency policies. Non-essential functions include participating in agency quality assurance programs, communicating information to other departments, assisting in policy development, and attending interdisciplinary team meetings for care coordination and problem-solving.
