About The Position

The Safe At Home Case Manager provides in home support for families involved in the Family Preservation program. The community/in home focus works to ensure the safety and wellbeing of both children and their families. This position is supervised by an Intact Supervisor. The SAFE At Home Case Manager is involved with families for an average of four (4) to six (6) months.

Requirements

  • Current, valid driver’s license and car insurance.
  • Reliable transportation to visit homes or other locations for assigned cases.
  • Be able to lift up to 25 pounds on a regular basis and up to 50 pounds on an occasional basis.
  • Above average verbal and written communication skills.
  • Ability to work collaboratively with a team.
  • Associates Degree in human services required

  • Bachelors degree in human services or related field preferred.

Responsibilities

  • Behavior management: Activities, arrangements, and services that seek to help direct and modify the caregiver’s behavior.
  • Crisis management: Specifically concerned with intervening to prevent or stop a crisis and to mobilize problem-solving to return a family to a state of calm.
  • Social connection: Seeks to help reduce social isolation and seeks to provide social support to families in assigned caseload.
  • Resource support: Assist parent(s) with connecting to areas resources for family support.
  • Child Care: Creates alternatives to the family routine, scheduling, demands, and daily pressure and provides designated, temporary, or longer-term periods of time when a child is not with the caregiver. Assists families with daycare or other caregiving enrollment and resources.
  • Safety Services: Provided predominately where a family lives and occasionally in the community depending on a family’s needs. The Case Managers observe and interact with children/adolescents weekly, and in some family situations multiple times per week (2 to 4 in-person contacts weekly) depending on the requirements of an in-home safety plan.
  • Safety Assessment: When interacting with children/adolescents, the Safety Case Managers: Assess their physical and emotional safety and needs of children in the home. Assist with meeting their basic or exceptional needs. Monitor their interactions with caregivers. Seek to increase their feelings of security.
  • Family Supports: Maintain contact with informal family supports who are involved in a safety plan. Maintain weekly contact with the Family Preservation caseworker regarding safety plan sufficiency.
