About The Position

Under the direction of the Program Director, this role is responsible for the overall data entry functions of the ARIES program. The position provides ongoing case management services to clients diagnosed with HIV/AIDS in Maverick, Val Verde, Uvalde, Dimmit, Zavala, Real, La Salle, Edwards, and Kinney Counties. Responsibilities include compiling client data, evaluating client needs, and coordinating social case management services with the Program Director. The role also manages various functions for administering the Ryan White and Social Service programs. The individual must demonstrate the knowledge and skills to provide age-appropriate care, understanding the needs of different age groups (infant, adolescent, geriatric). This position assists in health education and disease prevention within the community and served counties, promoting the "Health Promotion" program in the Tri County area. It supports the CEO/CMO in preparing and conducting classes, campaigns, and outreach activities, acting as a health motivator to encourage lifestyle changes in patients regarding diet and exercise. The role performs clerical functions, ensuring proper completion and handling of forms, and tracking program participation and attendance. Compliance with State/Federal regulations, PCHM Standards, and Compliance Standards for United Medical Centers is also required.

Requirements

High school graduate or equivalent.
Graduate from an accredited training program for Medical Assistants and/or Community Health Worker.
Certification or registration as Medical Assistant or Community Health Worker required.
Computer experience in different programs required.
Bilingual (English/ Spanish) required.
Ability to communicate proficiently with individuals of diverse backgrounds and economic levels.
Valid Texas Driver License and Liability Insurance coverage.
Good driving record.

Responsibilities

Overall data entry functions of the ARIES program.
Provide ongoing case management services to clients diagnosed with HIV/AIDS.
Compile client's data.
Evaluate client's needs.
Coordinate all social case management services with the Program Director.
Administer Ryan White and Social Service programs.
Provide care appropriate to the age of the clients served, demonstrating knowledge of growth and development principles.
Identify and categorize patient's age-specific grouping of needs.
Assist in prevention of health education and disease prevention in the community.
Promote the “Health Promotion” program in the Tri County area.
Assist the CEO/CMO during the preparation and conduction of classes, campaigns and outreach activities.
Serve as a health motivator, offering patients encouragement and support.
Influence patients to make lifestyle changes regarding diet and exercise.
Perform various clerical functions ensuring proper completion and handling of required forms.
Track program participation and attendance.
Ensure compliance with other requirements affecting United Medical Centers, such as State/Federal regulations, PCHM Standards and Compliance Standards.
