Under the direction of the Program Director, this role is responsible for the overall data entry functions of the ARIES program. The position provides ongoing case management services to clients diagnosed with HIV/AIDS in Maverick, Val Verde, Uvalde, Dimmit, Zavala, Real, La Salle, Edwards, and Kinney Counties. Responsibilities include compiling client data, evaluating client needs, and coordinating social case management services with the Program Director. The role also manages various functions for administering the Ryan White and Social Service programs. The individual must demonstrate the knowledge and skills to provide age-appropriate care, understanding the needs of different age groups (infant, adolescent, geriatric). This position assists in health education and disease prevention within the community and served counties, promoting the "Health Promotion" program in the Tri County area. It supports the CEO/CMO in preparing and conducting classes, campaigns, and outreach activities, acting as a health motivator to encourage lifestyle changes in patients regarding diet and exercise. The role performs clerical functions, ensuring proper completion and handling of forms, and tracking program participation and attendance. Compliance with State/Federal regulations, PCHM Standards, and Compliance Standards for United Medical Centers is also required.
