About The Position

The Ryan White Case Management Supervisor plays a critical leadership role in overseeing the delivery of comprehensive case management services to individuals living with HIV/AIDS under the Ryan White HIV/AIDS Program. This position ensures that clients receive high-quality, client-centered care that addresses their medical, psychosocial, and supportive service needs. The supervisor is responsible for guiding and supporting a team of case managers, fostering a collaborative environment that promotes professional development and adherence to program standards. They will also coordinate with healthcare providers, community organizations, and funding agencies to optimize service delivery and compliance with federal and state regulations. Ultimately, this role aims to improve health outcomes and quality of life for clients by ensuring effective case management and resource linkage within the community.

Requirements

  • Bachelor’s degree in social work, public health, psychology, or a related field.
  • At least three years of experience in case management or social services, preferably working with individuals living with HIV/AIDS.
  • Demonstrated supervisory or leadership experience in a healthcare or social service setting.
  • Knowledge of the Ryan White HIV/AIDS Program and its regulatory requirements.
  • Strong understanding of HIV/AIDS clinical care, psychosocial issues, and community resources.

Nice To Haves

  • Master’s degree in social work (MSW), public health (MPH), or a related discipline.
  • Experience working in a multidisciplinary healthcare team environment.
  • Familiarity with electronic health records (EHR) and case management software.
  • Bilingual proficiency, particularly in Spanish, to effectively serve diverse client populations.

Responsibilities

  • Supervise and mentor a team of Ryan White case managers, providing guidance, training, and performance evaluations to ensure high-quality service delivery.
  • Oversee client intake, assessment, care planning, and service coordination to meet the individualized needs of people living with HIV/AIDS.
  • Ensure compliance with Ryan White Program requirements, including documentation, reporting, and quality assurance standards.
  • Collaborate with healthcare providers, social service agencies, and community partners to facilitate access to medical care, housing, mental health services, and other supportive resources.
  • Develop and implement policies and procedures to improve case management practices and client outcomes.
  • Monitor program performance metrics and prepare reports for internal leadership and funding bodies.
  • Address and resolve complex client issues and barriers to care through problem-solving and resource coordination.
  • Participate in community outreach and education efforts to raise awareness of available services and support.
  • Manage individual case load to support delivery of Ryan White services.
