Requirements

• Minimum of 2 years’ of experience
• Must have National Certification
• Hold or be eligible to obtain an Arizona License and DEA
• Level One Fingerprint Clearance card (or receipt before hire date, card within 30 days)
• Undergo drug testing prior to employment
• Maintain vaccination status (recommended by CDC and/or other public health agencies) to include an Influenza vaccination (subject to exemptions and accommodations)

• Prefer working at an FQHC, CHC, or PCMH facility
