The North Carolina Rural Health Transformation Program (RHTP) is a statewide initiative led by the Department of Health and Human Services (DHHS) Rural Health Transformation Program Office and guided by a cross-agency steering committee. RHTP represents a projected $1B transformative investment to improve health outcomes and access to care for nearly 3 million rural North Carolinians. The position is responsible for providing outreach and navigator support for this program. The Rural Health Transformation Outreach Coordinator serves as a convener and navigator for Initiative 6 of the Rural Health Transformation (RHT) grant program. This position builds and maintains strong relationships with rural medical providers, local partners, residents, community stakeholders, the RHTP Technical Assistance Center, Cooperative Extension offices, NC 211, digital literacy hubs, and both grantees and subgrantees. The Lead ensures cohesive messaging, coordinated technical assistance, and unified service delivery across all components of the program.
