About The Position

The Registered Practical Nurse (RPN) - Footcare provides comprehensive foot care services to a diverse patient population, including individuals with diabetes and other chronic conditions. The RPN conducts foot assessments and delivers non-invasive foot care treatments such as nail trimming and corn/callus management. The role includes patient education to support self-management, as well as collaboration with Primary Care Providers and the interprofessional team to develop care plans, evaluate outcomes, and provide follow-up care.

Requirements

  • Registered Practical Nurse with a valid Certificate of Registration with The College of Nurses of Ontario (CNO) in good standing
  • Completion of Basic and Advanced Foot Care certification
  • Current CPR certification
  • Strong clinical skills and knowledge of current nursing practices, including assessment and management of foot health and chronic disease.
  • Proficiency with electronic medical records (EPIC or similar systems).
  • Excellent communication and interpersonal skills with a strong patient-centered and culturally safe approach to care.
  • Ability to work in a fast-paced environment, effectively prioritize competing demands, and maintain a high standard of accuracy and patient care.
  • Excellent critical thinking, clinical judgment, and decision-making skills within scope of practice.
  • Demonstrated ability to collaborate effectively within an interprofessional team.
  • Strong understanding of privacy, confidentiality, and documentation standards.
  • Knowledge of infection prevention and control (IPAC) practices.
  • Ability to provide patient education to support self-management and prevention.
  • Strong organizational and time management skills with the ability to work independently.

Responsibilities

  • Greet patients warmly and with compassion, ensuring an excellent patient experience.
  • Perform comprehensive foot assessments for patients, including those with diabetes and chronic conditions.
  • Provide non-invasive foot care treatments (e.g., nail trimming, corn and callus care).
  • Monitor patient outcomes and identify any concerns requiring escalation within scope of practice.
  • Educate patients on foot health, prevention strategies, and self-management techniques.
  • Promote patient independence and adherence to care plans.
  • Collaborate with Primary Care Providers and the interprofessional team to develop and implement care plans.
  • Participate in evaluating patient outcomes and adjusting care as needed.
  • Provide appropriate follow-up care and documentation.
  • Maintain accurate and timely documentation in EPIC.
  • Adhere to privacy, confidentiality, and organizational policies and standards.
  • Providers and the interdisciplinary healthcare team to coordinate patient care and ensure continuity of care
  • Participate in team meetings to discuss patient care plans and improve office processes.
