About The Position
Inspired Home Health is dedicated to providing compassionate and personalized in-home health care, ensuring the health, comfort, and independence of our clients in the comfort of their own homes. In this Home Health Nursing role, you will provide skilled in-home nursing care to patients across Phoenix, Arizona, supporting safe recovery, comfort, and independence. You will collaborate with physicians and the interdisciplinary team to implement and update individualized plans of care, monitor patient status, and escalate changes in condition in a timely, clinically appropriate manner. Accurate, compliant clinical documentation is essential, using a fully remote workflow to ensure continuity across settings. You will also deliver clear patient and caregiver education on medications, symptom management, and home safety to reinforce adherence and reduce risk.
Requirements
- Active Arizona RN in good standing
- Home health or comparable clinical experience
- Strong assessment, care-planning, and patient education skills
- Reliable documentation and time-management in a fully remote workflow
- Ability to support Weekends and/or Full-Time needs as scheduled.
- HCHB EMR experience
- Pass the OIG exclusion list
- Pass MVR
- Have an Arizona fingerprint clearance card
Responsibilities
- Provide skilled in-home nursing care to patients across Phoenix, Arizona.

- Organize route and priorities for Phoenix, Arizona.
- Connect with patients and caregivers, staying attentive to comfort, questions, and changing needs.
- Keep communication steady with the broader care team.
- Update records in a fully remote system between visits.
- Confirm upcoming appointments.
- Ensure orders and follow-ups are aligned.
- Provide coverage for weekends as needed to maintain dependable continuity.
