Requirements

• Graduate of an accredited physical therapy program with successful completion of the state licensure examination.
• Current, valid Physical Therapy license.
• Current CPR certification.
• Valid driver’s license and auto insurance.

Nice To Haves

• One year of home health experience preferred.
• HomeCare HomeBase experience preferred.

Responsibilities

• Perform and document initial evaluations to identify functional limitations and establish appropriate treatment plans.
• Develop and implement individualized therapy programs to improve physical function, promote independence, and prevent further decline.
• Monitor patient progress and adjust treatment plans to achieve the best possible outcomes.
• Educate patients and families on exercises, safe mobility, and strategies to support recovery.
• Collaborate with team members and other healthcare professionals to ensure continuity of care.
• Follow infection control practices and universal precautions during patient care.
• Complete accurate, timely documentation within company guidelines.
• Support agency philosophy and administrative policies.
