About The Position

We are seeking an experienced Wound Care Registered Nurse (RN) to provide specialized wound, ostomy, and continence care within a collaborative healthcare environment. The ideal candidate will conduct comprehensive patient assessments, develop individualized care plans, provide advanced wound management, and support patient and staff education while ensuring high-quality patient outcomes.

Requirements

  • Associate Degree in Nursing (ASN or ADN) from an accredited nursing program
  • Active New York State RN License
  • Primary Source Verification through NYSED
  • Current AHA BLS Certification
  • Certified Wound Ostomy Continence Nurse (CWOCN) – WOCNCB OR Certified Wound Ostomy Nurse (CWON) – WOCNCB OR Certified Wound Care Nurse (CWCN) – WOCNCB OR Certified Wound Specialist (CWS) – ABWM OR Wound Care Certified (WCC) – NAWCO
  • Minimum 2 years of Med/Surg nursing experience
  • Wound care and colostomy experience

Responsibilities

  • Conduct health assessments focused on wound, ostomy, and continence care
  • Perform focused physical assessments and develop individualized treatment plans
  • Recommend medications, treatments, and preventative interventions as appropriate
  • Assess, monitor, and document patient responses to wound care and complex treatment plans
  • Manage care for wounds, fistulas, ostomies, continence issues, and pressure injuries
  • Collaborate with interdisciplinary healthcare teams and initiate referrals when necessary
  • Provide patient, family, and caregiver education regarding wound management and prevention of complications
  • Develop, implement, and evaluate staff education and continuing education programs
  • Support quality improvement initiatives, prevalence studies, and nursing quality programs
  • Assist with the selection and evaluation of wound care supplies, equipment, and technologies
  • Monitor and manage specialty equipment including specialty beds and negative pressure wound devices
  • Maintain compliance with patient confidentiality, infection prevention, and documentation standards
  • Identify and coordinate community resources to support patient care and recovery
  • Promote culturally competent patient-centered care in accordance with patient needs and belief systems
