About The Position
Home Health Care, Inc. is seeking an RN Visit Nurse for the St. Paul territory. This role offers the opportunity to provide 1:1 patient care in patients' homes, helping them recover after hospitalization, manage chronic illnesses, avoid unnecessary readmissions, and remain independent. The position combines the autonomy of field work with the support of an established home health organization. Nurses will perform comprehensive nursing assessments, deliver various treatments, educate patients and caregivers, coordinate care, monitor progress, and complete documentation. The goal is to help patients remain safely at home and reduce avoidable hospitalizations.
Requirements
- Active Minnesota Registered Nurse (RN) license
- Experience in home health, hospital, rehabilitation, skilled nursing, or acute care
- Strong clinical judgment and assessment skills
- Excellent communication and patient education abilities
- Self-motivated and comfortable working independently
- Reliable transportation and willingness to travel throughout the St. Paul service area
Responsibilities
- Provide skilled RN home health visits throughout the St. Paul territory
- Perform comprehensive nursing assessments
- Deliver wound care, medication management, injections, and disease-specific treatments
- Educate patients and caregivers on medications, diagnoses, and recovery plans
- Coordinate care with physicians, therapists, and interdisciplinary team members
- Monitor patient progress and communicate changes in condition
- Complete accurate documentation using Homecare Homebase
- Help patients remain safely at home while reducing avoidable hospitalizations
