About The Position

The Role As a PRN RN with StellarCare, you will provide high-quality home health nursing services to patients in the comfort of their homes. You pick the coverage areas and the days you want to work—we handle the rest.

Requirements

• Credentials: Graduate of an accredited nursing program with a current, unrestricted Registered Nurse (RN) license in the state of practice.
• Experience: Minimum of 1 year of nursing experience required; Home Health experience preferred.
• Reliability: Valid driver's license, current auto insurance, and dependable transportation.
• Tech-Savvy: Comfort with electronic documentation, mobile communication, and EMR systems.
• Clinical Knowledge: Strong assessment, critical thinking, and patient education skills with the ability to work independently in a home health setting.

Nice To Haves

• Home Health experience preferred.

Responsibilities

• Deliver Exceptional Patient Care: Conduct comprehensive nursing assessments and develop individualized plans of care that promote health, independence, and quality of life.
• Clinical Excellence: Perform skilled nursing visits, including medication management, wound care, disease management education, and patient/caregiver instruction.
• Care Coordination: Monitor patient progress, communicate changes in condition, and collaborate with physicians, therapists, and interdisciplinary team members to ensure optimal outcomes.
• Documentation: Complete timely and accurate OASIS assessments, nursing notes, care plans, and other required documentation through our EMR system.
• Patient Advocacy: Educate patients and families on diagnoses, treatments, medications, and self-care strategies while promoting safety and independence at home.
