About The Position

Home Health Care, Inc. is hiring a Full-Time RN Visit Nurse to serve patients throughout St. Paul and surrounding communities. This role focuses on providing one-on-one patient care in their homes, allowing for focused time with each patient to assess their condition, administer skilled care, educate families, and coordinate with physicians and other healthcare providers. The position offers independence and the opportunity to build relationships with patients and families, contributing to better health outcomes by preventing avoidable emergency room visits and hospitalizations. The nurse will travel throughout the St. Paul territory, completing comprehensive nursing assessments, wound care, medication management, and other skilled nursing procedures, while documenting care in Homecare Homebase (HCHB).

Requirements

  • Active Minnesota Registered Nurse (RN) license
  • Previous nursing experience in home health, hospital, rehabilitation, skilled nursing, or another clinical environment
  • Strong assessment and critical-thinking skills
  • Confidence working independently in the field
  • Excellent patient and family communication
  • Strong organization and time-management skills
  • Ability to collaborate effectively with an interdisciplinary team
  • Reliable transportation and willingness to complete visits throughout the St. Paul territory

Nice To Haves

  • Previous home health experience is valuable, but we're also interested in experienced nurses from other settings who are ready to bring their clinical skills into the home.

Responsibilities

  • Travel throughout the St. Paul territory, providing skilled nursing visits to patients recovering or managing their health at home.
  • Conduct comprehensive nursing assessments.
  • Perform wound assessment and wound care.
  • Conduct medication reconciliation and management.
  • Administer injections and other skilled nursing procedures.
  • Monitor acute and chronic medical conditions.
  • Provide patient and caregiver education.
  • Identify and communicate changes in condition.
  • Coordinate with physicians and other providers.
  • Collaborate with PT, OT, SLP, social work, and other members of the care team.
  • Help prevent avoidable emergency room visits and hospitalizations.
  • Complete timely documentation in Homecare Homebase (HCHB).
