About The Position
Home Health Care, Inc. is seeking a dedicated RN Visit Nurse to provide 1:1 patient care in homes, assisted living, and community settings. Unlike other agencies that may stretch nurses thin or leave them unsupported, we have built our agency differently with a focus on supporting the nurse so they can focus on the patient. Requirements
- Active Minnesota Registered Nurse (RN) license.
- Experience in home health, med-surg, rehab, or clinical nursing.
- Ability to work independently while staying connected to a team.
- Strong communication and patient education skills.
- Reliable transportation and willingness to travel locally.
Responsibilities
- Provide skilled nursing visits in patient homes and care settings.
- Deliver hands-on care including wound care, medication management, and assessments.
- Build consistent relationships with patients and families.
- Coordinate care with physicians, therapists, and interdisciplinary teams.
- Educate patients and caregivers on care plans and recovery goals.
- Complete accurate, timely documentation.
