About The Position

Home Health Care, Inc. is seeking an RN Visit Nurse to provide skilled nursing care to patients in the St. Paul area and surrounding communities. Requirements

• Active Minnesota Registered Nurse (RN) license
• Experience in home health, acute care, rehabilitation, long-term care, or skilled nursing
• Strong clinical assessment and decision-making skills
• Excellent communication and patient education abilities
• Self-motivated with strong organizational skills
• Reliable transportation for local travel throughout the St. Paul service area

Responsibilities

• Provide skilled RN home visits throughout the St. Paul territory
• Complete comprehensive patient assessments
• Perform wound care, medication management, injections, and other skilled nursing procedures
• Develop and update individualized care plans
• Educate patients and caregivers on medications and disease management
• Communicate with physicians and interdisciplinary care teams
• Complete timely documentation using Homecare Homebase
• Monitor patient progress and coordinate ongoing care
