About The Position
Home Health Care, Inc. is seeking a dedicated RN Visit Nurse to provide 1:1 patient care in patients' homes. For over 30 years, we have been a leading locally owned Medicare-certified home health agency in Minnesota, committed to providing high-quality care and investing in our clinicians. Requirements
- Active Minnesota Registered Nurse (RN) license
- Previous nursing experience in home health, hospital, rehabilitation, skilled nursing, or clinical care
- Strong assessment and critical thinking skills
- Ability to work independently while collaborating with an interdisciplinary team
- Excellent communication and organizational skills
- Reliable transportation and willingness to travel throughout the local service area
Responsibilities
- Provide skilled home health nursing visits throughout your assigned territory
- Complete comprehensive patient assessments
- Perform wound care, medication management, injections, and other skilled nursing services
- Monitor patient progress and communicate changes to physicians
- Educate patients and caregivers on disease management and treatment plans
- Coordinate care with therapists, social workers, and interdisciplinary team members
- Complete timely documentation within our Homecare Homebase (HCHB) EMR
- Help reduce hospitalizations through proactive, high-quality patient care
