Registered Nurse – In -Home Visits, Supportive Team, $4K Bonus
About The Position
This role offers a different approach to nursing compared to hospital, TCU, or SNF settings, which often have high patient ratios, constant interruptions, and limited time with each patient. As an RN Visit Nurse, you will provide true one-on-one, in-home patient care, building relationships and delivering meaningful outcomes in a setting that allows you to slow down and focus. This position offers full-time stability with consistent scheduling, autonomy in the field with strong clinical support, and the opportunity to see patients improve and regain independence. You will be backed by a 30+ year home care leader in Minnesota.
Requirements
- Active Minnesota Registered Nurse (RN) license.
- Experience in home health, med-surg, TCU, rehab, or clinical nursing.
- Strong clinical judgment and ability to work independently.
- Excellent communication and patient education skills.
- Valid driver’s license and reliable transportation.
Responsibilities
- Provide skilled nursing visits in patient homes and care settings.
- Deliver treatments including wound care, medication management, and assessments.
- Coordinate care with physicians, therapists, and interdisciplinary teams.
- Educate patients and families on care plans, medications, and recovery goals.
- Maintain accurate, timely documentation in the EMR.
