About The Position
This role offers a different approach to nursing compared to fast-paced hospital, TCU, or SNF settings. As a RN Visit Nurse, you will provide true one-on-one, in-home patient care, building relationships and delivering meaningful outcomes in a setting that allows you to slow down and focus. You will have the autonomy to work in the field with strong clinical support, backed by a company with over 30 years of experience in home care. This is an opportunity to see patients improve and regain independence.
Requirements
- Active Minnesota Registered Nurse (RN) license.
- Experience in home health, med-surg, TCU, rehab, or clinical nursing.
- Strong clinical judgment and ability to work independently.
- Excellent communication and patient education skills.
- Valid driver’s license and reliable transportation.
Responsibilities
- Provide skilled nursing visits in patient homes and care settings.
- Deliver treatments including wound care, medication management, and assessments.
- Coordinate care with physicians, therapists, and interdisciplinary teams.
- Educate patients and families on care plans, medications, and recovery goals.
- Maintain accurate, timely documentation in the EMR.
