About The Position

The Suites of Someren Glen is seeking a strong nurse leader with experience in rehab and skilled nursing to serve as our Neighborhood/Unit Manager. This role is a key member of our nurse leadership team, responsible for the oversight, training, and mentorship of care partners throughout the Community. We're looking for someone who leads with collaboration, follows through on the details, and is genuinely invested in the quality of care our residents receive. As Neighborhood/Unit Manager, you'll guide the day-to-day operations of nursing care within your unit, ensuring established policies and programs are followed and that PPS and MDS compliance is maintained. You'll coordinate scheduling and personnel management, tailor care to each resident's individual needs and preferences, and play a key role in selecting, training, and mentoring CNAs, LPNs, and RNs.

Requirements

  • Active RN license in good standing
  • Experience in rehab and skilled nursing required
  • Strong working knowledge of PPS and MDS processes
  • Excellent communication, coaching, and organizational skills
  • Ability to remain calm and decisive during clinical emergencies
  • Willingness to participate in on-call rotation
  • Licensed Registered Nurse in the State of practice; BSN preferred.
  • Active CPR certification.
  • Minimum of 2 years work experience in a Skilled Nursing Community.
  • 1-2 years’ experience supervising and managing direct reports, Proven ability to effectively manage staff.
  • Knowledge of Health Department regulations including the annual Federal and State Health Department survey process including PPS, MDS, and OBRA requirements preferred.
  • Must be able to demonstrate competency in Microsoft Office, Windows, and other applicable software programs relevant to the job.
  • Must be able to read, write and speak the English language.
  • Must possess excellent customer service skills.

Responsibilities

  • Monitors residents' changing needs, ensures timely interventions, and communicates updates to the care team through daily rounds
  • Conducts thorough pre-admission assessments to identify resident needs before move-in
  • Evaluates and implements new tools and technology to enhance resident care
  • Serves as a go-to resource for staff during clinical emergencies and changes in resident condition
  • Performs regular compliance audits to ensure quality and regulatory standards are met
  • Coordinates care plans and facilitates patient care conferences with the interdisciplinary team
  • Oversees MDS compliance within the unit
  • Ensures admission documentation is complete, accurate, and meets company standards
  • Recognizes signs and symptoms of resident conditions and exercises sound clinical judgment in determining appropriate treatment or next steps
  • Serves as a key communicator between nursing staff, physicians, pharmacists, residents, and families
  • Participates in on-call rotation as needed
  • Partners with the Director of Nursing to interview, develop, and guide frontline staff
  • Other duties as assigned
