About The Position

Sky View Rehabilitation and Health Care, a 192-bed Skilled Nursing Facility located in Croton-on-Hudson, NY, has an opening for a Registered Nurse Unit Manager, RN. Sky View serves as the bridge between hospital and home, spanning the gap from recovery to community reentry. Our luxurious and hotel-like environment is the ideal setting for those in need of Post Acute Rehabilitation or Long-Term Care. Our team of highly skilled physical, occupational, speech, and respiratory therapists really care and work together to create an individualized therapy plan that helps residents reach their highest level of functioning in as short an amount of time as possible – preparing for their eventual return home. The Registered Nurse Unit Manager, RN performs professional supervision, guidance, and direction to staff in both clinical and personnel issues on the Sub-Acute/Short Term Unit. This role coordinates resident care in accordance with State DOH regulations, provides support to residents/families in dealing with clinical issues and challenges, and ensures staff adheres to safety and infection control guidelines. The position assists in establishing daily operations of resident care on the unit, including showers/baths, toileting, meals, medication administration, and activities. Communication with the Interdisciplinary Team is crucial to assure resident/family needs are met. Daily rounds of all residents on the unit are performed, along with assessments (Admission, Elopement, Pain, etc.), and the development of a plan of care for each resident. Documentation and communication of the plan of care on Resident Information Sheets are required. The role ensures clinical observation and assessment documentation on all assigned residents per regulations, reports findings to the designated Nursing Supervisor, and documents the effectiveness of the plan of care. Preparation of resident assignments for Certified Nursing Assistants, considering resident needs and personnel competency, is also a key responsibility. Ongoing resident, family, and facility plans/concerns/information are communicated to staff. The RN Unit Manager organizes, oversees, and supervises assigned unit activities and staff assignments, provides Nursing Assistant assignments, and communicates pertinent resident care needs. Monitoring and evaluating the flow of activity and delivery of patient care provided by CNAs (turning and positioning, mealtimes, bathing, etc.) is essential. Ensuring CNA documentation records (ADLs, BM, Meal intake, etc.) are completed is also part of the role. The RN Unit Manager communicates pertinent resident findings to Interdisciplinary Team members, assists other staff members in completion of their duties, and orients new Nursing personnel as assigned. Other related duties as required and assigned.

Requirements

Must be a graduate of an approved school of nursing.
Minimum Bachelor’s Degree in Nursing or three (3) years’ experience in long term care
Current licensure in NYS as a Registered Professional Nurse or Licensed Practical Nurse
Minimum three (3) years’ experience in a supervisory capacity either in a hospital, long term care facility, or other related medical facility
Training in rehabilitative and restorative nursing practice.
Must have a sincere desire to work with the elderly, chronically ill, and disabled.
Must have a thorough knowledge of medical, social, and psychological needs of residents.
Must utilize good judgment in emergency situations.
Must possess a spirit of cooperation and enthusiasm in order to create an atmosphere conducive to rehabilitation and growth.
Must be able to communicate well verbally with residents, families and staff.
Must be able to handle emergencies with calm and intelligent response.
Must have computer literacy.

Responsibilities

Perform professional supervision, guidance and direction to staff in both clinical and personnel issues on Sub-Acute/Short Term Unit.
