About The Position

Responsible for the coordination of the care and management of the referrals into advanced organ disease programs and pre-transplant candidate. This includes the initiation and completion of the evaluation, coordination, and timing of testing to determine candidacy for transplant or advanced therapies. Must be able to function with considerable independence in the management of the referrals and pre-transplant patient. Responsible for promoting the health and safety by ensuring the ongoing communication and collaboration providers and other multidisciplinary team members.

Requirements

  • Licensed or eligible for license in Kentucky
  • Basic Life Support (BLS) accredited by the American Heart Association (AHA)
  • Associate’s degree from accredited college or University
  • Four years of experience in a clinical setting

Responsibilities

  • Coordinates referrals and follow-up evaluation of each patient.
  • Collaborates with the multidisciplinary to assure appropriate evaluation per protocol and policies.
  • Is educated and understands general management protocols for organ specific evaluation.
  • Schedule laboratory tests, X rays, biopsy, etc. according to the established protocols.
  • Monitors laboratory results for each patient and knows normal values for the labs, follows patient trends. If abnormal result or trend noted, coordinator responds per protocol or notifies transplant provider of abnormalities and coordinators follow up as directed.
  • Promptly notifies patient (and consultants and other treaters) when patient care issue noted
  • Phone triage of patient calls and MD calls with ongoing education to patients, shares information regarding protocols and general knowledge with PCP’s or other community treaters.
  • Communicates with other team members problems that are occurring so that continuity of care is maintained.
  • Communicates the results of lab tests and biopsies to the patient and their families.
  • Communicates on an ongoing basis with patients.
  • Documents in Transplant Data Base and appropriate hospital and clinic EMR’s conversations with patient, uses To-Do function.
  • Works with multidisciplinary team members in assuring that the psychosocial and financial needs of the patient are being addressed.
  • Maintains accurate and current data base on each patient recording all laboratory, diagnostic tests, biopsy results and changes in medications.
  • Staffs the outpatient transplant clinic.
  • Schedules all clinic visits according to established protocols and the needs of the patients.
  • Staffs the outpatient transplant clinic with providers and other members of multidisciplinary team, prepares nursing assessment, prepares appropriate documents and reports for clinic and documents clearly each encounter when patient seen in clinic (changes to medications, procedures to be scheduled for follow up on, laboratory schedules changes, etc.).
  • Schedules all clinic visits according to established protocols and the needs of the patients.
  • Discuss findings with the transplant provider and appropriate members of multidisciplinary team and carries out the follow-up that is indicated.
  • Maintain compliance with all company policies, procedures and standards of conduct.
  • Active involvement in program specific quality and selection meetings, protocol development, and educational programs for community, patients, and staff.
  • Other duties as assigned, including but not limited to cross coverage training.
