About The Position

Fairview is looking to hire an RN Transplant Coordinator to join our Post Transplant Kidney/Pancreas Team at the University of Minnesota campus in Minneapolis. This is an exciting opportunity for an RN who enjoys building long-term relationships with patients, coordinating complex care, and working as part of a highly collaborative transplant team. This is a full-time, 80 hours per pay period, benefit-eligible, exempt position offering a flexible hybrid schedule. Team members work onsite two days per week and from home three days per week. Wednesday is a designated onsite day, with the second onsite day rotating based on clinic coverage needs. As an RN Transplant Coordinator, you will play an important role in supporting kidney and pancreas transplant recipients throughout their post-transplant journey. You’ll partner closely with patients, families, physicians, advanced practice providers, and other members of the multidisciplinary transplant team to coordinate care, provide education, triage patient needs, and help patients successfully navigate life after transplant. This role is a great fit for an RN looking to use their clinical knowledge in a specialized setting while enjoying a Monday–Friday schedule, hybrid work flexibility, limited call responsibilities, and the opportunity to make a lasting impact on transplant patients and their families. The Transplant Coordinator is responsible for coordination and preparation of patients for organ transplantation and the long-term management of patients after transplant through collaboration with various members of the interdisciplinary team.

Requirements

  • Bachelors Degree in Nursing or Associates Degree in Nursing
  • If Bachelors Degree: 3-5 years acute care experience in ICU, CCU, Transplant, Acute Care Med-Surg or If Associates Degree: 5+ years of acute care experience in ICU, CCU, Transplant, Acute Care Med-Surg
  • MN Registered Nurse (RN) License
  • Basic Life Support (American Heart Assoc or Red Cross)

Responsibilities

  • Coordinate patient care in the post-transplant phases
  • Collaborate with interdisciplinary team members within the organization and outside community partners
  • Act as a liaison for transplant physicians to implement medical management of conditions
  • Provide telephone triage and make patient care decisions based on patient symptom assessment and evaluation, laboratory results, and patient condition changes
  • Apply appropriate medical interventions and transplant protocols based on patient assessment, laboratory results, and patient condition changes
  • Assumes on-call responsibilities after office hours by providing a resource for medical triage. Acts as transplant coordinator when donor organ becomes available.
  • Provides on-going education to patients and families
  • Collaborate with other members of the clinical team to improve patient care and benchmarks through quality improvement projects
  • Participate in the development and revision of protocols
  • Maintain knowledge of current practices in transplantation, nursing, and healthcare
