About The Position

Sentara Health is seeking an RN for its Observation/Transitional Care Unit. The Obs/Transitional Care Unit is a 9-bed short stay unit used to care for med-surg patients who will be discharged within 24-48 hours. This is a Flexi position located in Norfolk, VA, with a First (Days) work shift.

Requirements

RN Licensure - Virginia Required
BSN, RN-Associate's Degree OR RN-Diploma Required
Two years of nursing experience

Responsibilities

Assesses, plans, develops, implements, and evaluates plans of care for specific patient populations.
Coordinates and collaborates with a multidisciplinary team to facilitate appropriate and timely patient placement and/or transfer.
Coordinates and communicates capacity management plan to nursing units, leadership team, and physicians.
Possesses clinical knowledge and skills to meet standards of care as required by patient populations.
