About The Position
Senior Helpers is a leading provider of compassionate, high-quality home care services across the Memphis area. We support patients in their recovery by delivering tailored care in the comfort of their homes. We are seeking a highly motivated RN with a heart for patient advocacy and a passion for building professional relationships to join our growing team.
Requirements
- Currently licensed as an RN
- Strong interpersonal and communication skills
- Highly organized, self-motivated, and goal-oriented
- Valid driver’s license and reliable transportation
Nice To Haves
- Experience in discharge planning or case management strongly preferred
Responsibilities
- Build and grow relationships with hospitals, rehab centers, the VA and assisted living facilities
- Collaborate with hospital and rehab discharge planners, case managers, and physicians to identify patients suitable for home care.
- Provide education to patients and families about our home care services.
- Conduct clinical assessments to determine eligibility and service needs.
- Represent Senior Helpers within the local senior community at events and programs
