About The Position

The RN Supervisor is responsible to maintain the nursing philosophy, objectives, and standards of quality nursing care; and ensure compliance with standards, codes, rules and regulations, and the policies set-forth by Woodland Pond. This role involves active participation in interdisciplinary team (IDT) meetings, ensuring resident rights are maintained, supporting medical staff, and collaborating with all departments. The RN Supervisor oversees the creation of assignments for LPNs, CNAs, and UAs, ensures accurate documentation, and provides a full report to the Charge RN. They are responsible for the accuracy and timeliness of care plans, Kardex, MDS, and assessments, and ensuring the care plan is followed. Communication with residents, families, staff, MDs, and IDT is crucial. The RN Supervisor may assist with performance counseling and evaluations, coordinate education, and ensure staff compliance with policies and standards of care. They also manage equipment and supplies, ensure staff awareness of policy and procedure manuals, and strive to enhance the resident and staff experience through quality assurance and safety protocols. Staffing coverage, including working beyond normal hours and taking on-call duties, is also a responsibility. The role requires patience, tact, enthusiasm, and the ability to work harmoniously with others, promoting a positive and calm environment. Ongoing education and adherence to evidenced-based nursing practices are encouraged. Enhanced responsibilities include addressing family/visitor/resident companion concerns, working closely with medical staff during rounding and for changes in resident condition, performing and ensuring thorough admissions and discharges (including order entry, advanced directives, MOLST forms, clinical assessments, physical assessments, and resident/family education), coordinating follow-up appointments, attending IDT meetings, and managing resident appointments and activities. They also assist with investigations, attend wound care rounds, manage bowel protocols, review labs and diagnostic testing, communicate changes to medical staff and families, and assist with meals to ensure resident needs are met.

Requirements

  • RN graduate of an accredited school of nursing
  • Minimum two years of clinical nursing experience preferred
  • Current, unencumbered RN license to practice in the State of New York
  • Proficiency with computers and standard office software
  • Valid CPR certification
  • Must be able to adapt to resident status and needs, as directed.
  • Schedule changes daily due to staffing, resident condition, new residents, etc.
  • Must be on-call

Responsibilities

  • Serve as a resident advocate and ensure resident rights are maintained.

  • Ensure safe quality care is provided to the residents daily, and essential information is communicated for appropriate continuity of care from admission to discharge.
  • Act as a liaison with residents, families, and other departments to adequately plan for and provide resident care.
  • Oversee the creation of assignments for the LPNs, CNAs, UAs.
  • Ensure accurate, appropriate, and complete documentation by all nursing staff.
  • Review MD progress notes to ensure accuracy and completion, following in-house visits.
  • Ensure a full and thorough report is given to the Charge RN (Nights), including tasks that need to be carried over to the next shift.
  • Ensure accuracy of each Care Plan, Kardex, MDS, and Assessment.
  • Complete timely assessments as assigned.
  • Effectively communicate the plan of care.
  • Ensure that resident care plan is followed.
  • Follow the Weekly RN assessment schedule.
  • Ensure appropriate and ongoing communication to the resident, family, staff, MD, IDT, etc.
  • Use communication to enhance quality of resident care and staff working environment.
  • Communicate with unit manager regularly about the status of the unit, staff, and residents.
  • May be asked by the unit manager to assist with pc&ds and scheduled evaluations.
  • Actively communicate with the unit manager and staff educator to coordinate educations for personnel.
  • Promote an environment of learning.
  • Ensure staff attends mandatory in-services and meets minimum continuing education requirements.
  • May be asked by UM or staff educator to assist with staff audits, to ensure care is being delivered per care plan and medical staff orders.
  • Ensure work consistency among staff.
  • Promote a positive, friendly work environment, leading by example.
  • Provide staff with praise and also re-educations as necessary.
  • Ensure appropriate equipment and supplies are available and monitor that supplies are utilized appropriately to avoid waste.
  • Be cost-effective.
  • Ensure staff awareness of manuals, including other departments (e.g. SDS, emergency protocols).
  • Use manual and assist other staff on using policies as necessary.
  • Constantly strive to enhance the resident and staff experience.
  • Report and complete necessary paperwork for each resident incident/accident/change in condition.
  • Ensure staff compliance with safety and infection control standards and regulations.
  • Report potential outbreaks and take appropriate measures to control/eliminate infection control exposures.
  • Initiate and complete appropriate sections of employee I&As when they occur.
  • Work beyond normal working hours, or in other positions temporarily when necessary (e.g. administers medications).
  • Be subject to callback during emergency conditions.
  • Take RN on-call as needed.
  • Accept and record call outs on staffing call out forms.
  • Communicate call out to staffing coordinator, UM, and DON.
  • Provide return to work instructions per policies guided by the NYSDOH and CDC, depending on the reason for the call out.
  • Address the high volume of family, visitor and resident companion questions and concerns during high visitation times.
  • Work closely with medical staff for in house visits, during medical staff rounding, several days each week.
  • Provide assessment, data and insight to medical staff both in house and on call for any change in resident condition that may require order changes in medication, treatment etc.
  • Perform and/or ensure the completion of in-depth and thorough admissions and discharges.
  • Ensure advanced directives are in place or are completed via a MOLST at the time of admission.
  • Complete and/or ensure a MOLST form is done in the absence of social work.
  • Assure communication of all follow up consult appointments to Medical Services Assistant.
  • Complete all Clinical Assessment forms for each admission at the time of admission.
  • Responsible for relaying incomplete admission tasks to the Charge RN (Nights).
  • Conduct a full Physical Assessment of Skin and every body system and document such.
  • Complete an in-depth interview with new resident and family, as well as review of medical record to ensure an accurate, complete admission.
  • Educate resident and family to the facility policies and procedures.
  • Complete all nursing instructions for CNAs and Unit Assistants.
  • Communicate with medical staff re: the new admission orders and diagnoses.
  • Complete Discharge Summary with all orders-Medications, Treatments, Therapy orders and Equipment needed).
  • Provide any needed discharge teaching and document each instance in the medical record.
  • Write all scripts for discharge.
  • Complete Form 3122 for all discharges to the Enhanced units.
  • Complete an O2 necessity form if necessary for discharge.
  • Attend Daily IDT meeting each morning to review each residents progress.
  • Review of all resident outgoing appointments that day.
  • Coordinate with CNAs and Medical Services Assistant to ensure residents make each appointment and that appropriate documentation goes with the resident to their appointment.
  • Upon return the day charge nurse must review the consult recommendations, communicates those needs to the provider and enter any needed order or care related changes in the EMR.
  • Ensure all residents get to in house and out-of-house appointments.
  • Daily review of appointment calendar.
  • Ensure all residents get to planned activities.
  • Ensure all residents get to therapy appointments in house.
  • Assist with completion of all Investigations and Analyses (I&As) not completed on the prior shift.
  • Attend weekly Wound Care Rounds as part of that team.
  • Perform daily follow up and documentation in the EMR for any resident requiring Bowel Protocol intervention and medications with an RN assessment.
  • Review all labs and diagnostic testing reports performed on shift and all pending labs from previous shifts/days.
  • Communicate those results with medical staff and enter any needed order changes in the EMR as well as a progress note about the visit and changes.
  • Communicate with Family/Health Care Proxys ANY changes in medications, resident conditions, consult results from appointments, and incident especially those resulting in an I&A (skin injuries, fall, behaviors, etc.).
  • Attend care plan meetings as needed.
  • Assist with all meals to ensure that appropriate assistance is provided to all residents and nursing coverage is as care planned.
