About The Position
Apple Rehab Middletown is a 70-bed skilled nursing facility located in Middletown, CT, near Route 9 and downtown. As a family-owned and operated company, Apple Rehab prioritizes treating residents and staff like family, with local senior management ensuring a supportive environment. The facility hosts special events and holiday activities, fostering a strong sense of community among staff, residents, and their families. This role involves planning, overseeing, and executing nursing services, supervising daily nursing activities, and collaborating with the Nursing Director on shift operations and emergency support. The RN Supervisor coordinates nursing activities, manages charge nurses, adjusts care plans for emergencies, and assumes full responsibility in the absence of superiors. They also ensure adequate relief staff and notify physicians of significant resident condition changes.
Requirements
- Current state RN license.
- Registered Nurse in good standing.
- Previous experience in a supervisory capacity.
- Attention to detail, good follow through skills and ability to prioritize multiple tasks.
- Cooperation in assisting others and ability to work as a team.
- Knowledgeable of general rehabilitative and restorative nursing and medical practices, procedures, laws, regulations and guidelines governing long term care.

- Long term care experience preferred.
Responsibilities
- Planning, overseeing, and executing nursing services for residents while ensuring compliance with local, state, and federal regulations.
- Supervising day-to-day nursing activities to guarantee residents receive top-quality care consistently.
- Collaborating with the Nursing Director to strategize nursing service operations and emergency support provisions for shifts.
- Coordinating nursing activities in alignment with medical requirements, physician directives, and general care plans.
- Managing and directing charge nurses to ensure residents receive proper care.
- Adjusting care plans in collaboration with medical staff to address emergency medical needs.
- Assuming total responsibility for operating decisions required by the circumstances of the moment in the absence of superiors.
- Ensuring adequate relief staff availability before leaving the facility.
- Notifying residents' personal physicians in case of significant changes in their condition or immediate medical care needs.
