About The Position

Focused Post Acute Care Partners (FPACP) is seeking a dedicated RN Supervisor to join their team at Focused Care at Cedar Bayou in Baytown, TX. This role supports the Director of Nursing in supervising nursing services and ensuring a high standard of care for residents, addressing their medical, physical, emotional, and psychological needs. FPACP fosters a positive culture with a recognition program called ROCKStar (Recognizing Outstanding Care and Kindness), which offers a gamified experience for team members to level up and potentially attend an annual gala.

Requirements

A registered nurse licensed and in good standing with the Texas State Board of Nurse Examiners in the state of Texas.

Responsibilities

Supports the Director of Nursing in monitoring, evaluating, and organizing direct nursing care.
Effectively communicates with the Director of Nursing, Medical Doctors, team members, and residents to promote optimal clinical outcomes and continuity of care.
Works cooperatively as part of the interdisciplinary team to assess, develop, implement, and evaluate resident care plans.
Makes frequent rounds throughout the shift and assists residents and staff as needed.
Utilizes critical thinking for resident conditions and implements interventions to prevent avoidable rehospitalizations.
Monitors the completion of electronic medical records/documentation.
Educates staff on policies and processes as necessary.
Supports newly hired nursing staff.
