About The Position

Focused Post Acute Care Partners (FPACP) is seeking an RN Supervisor to join their team in Humble, TX. FPACP emphasizes excellence in long-term care and has a recognition program called ROCKStar (Recognizing Outstanding Care and Kindness). This program uses gamification to motivate team members and offers a chance to attend an annual Gala. The RN Supervisor, under the Director of Nursing, will assist in supervising nursing services, setting care standards, and ensuring residents receive comprehensive medical, physical, emotional, and psychological care.

Requirements

A registered nurse licensed and in good standing with the Texas State Board of Nurse Examiners in the state of Texas.

Responsibilities

Supports the Director of Nursing in monitoring, evaluating, and organizing direct nursing care.
Effectively communicates with the DON, MD, team members, and residents to promote optimal clinical outcomes and continuity of care.
Works cooperatively as part of the interdisciplinary team to assess, develop, implement, and evaluate resident care plans.
Makes frequent rounds throughout the shift and assists residents and staff as needed.
Utilizes critical thinking for resident conditions and implements interventions to prevent avoidable rehospitalizations.
Monitors the completion of electronic medical records/documentation.
Educates staff on policies and processes as necessary.
Supports newly hired nursing staff.
